Case Managers are essential to getting an individual the adequate support needed. They are essential to all teams we support.

A case manager is responsible for advocating which supports would benefit the individual and assisting them in finding these supports. A case manager will provide an individual a “Pick List”. On the list, it shows all providers that have the support the individual is searching for. Case managers can assist individuals in the process of selecting a provider to choose.

Case managers are also responsible for maintaining an individual’s budget. Outside of the individual and/or guardian, the case manager is the person solely responsible for ensuring an individual receives the supports needed while staying within their Medicaid budget. A case manager is the person who updates the Person-Centered Individualized Support Plan (PCISP) and Service Authorization (SA). The PCISP will have all information pertaining to an individual, all goals for an individual, and all supports that an individual receives in it. A Service Authorization will have the units allotted to each support in it.
