Individuals may have risk plans if they have additional needs/risks that staff should be aware of. The type of risk plans the individual is assigned is determined by the individualized support team during the annual planning meeting. Risk plans may be implemented if the individual has seizures, a cardiac condition, allergies, etc. Think of it as an additional training to get to know your individual better and to provide them the best support possible. The state requires Assisted Independence to update risk plans annually and for Direct Support Professionals to read and sign risk plans annually.
