Cancellation Forms are an asset to help us keep track of cancelled sessions with all of our individuals. We are to complete an absence form any time a session is cancelled by an individual.

Step 1: On the left-hand side, select the plus sign next to forms.

Step 2: Select the Form: Absence Form

Step 3: Once the form is selected, it will go to this screen. To complete the form, you must choose a consumer from the list, select the date of service, time of service, service cancelled, and a note for the cancellation. The time of service should be when the session would have begun. The note should be why the individual cancelled the session. The submit button should be a royal blue in color to submit the form.
