Functional Assessment of Characteristics for Therapeutic Recreation – Revised (FACTR-R).
For individuals over the age of 10 years old, Assisted Independence requires the FACTR-R AX.
The assessment should be completed initially and once annually. The scores should improve from year to year. The FACTR-R Instruction Sheet will assist you in completing each domain of the assessment.

The FACTR-R focuses on the domains of Physical, Cognitive, and Social/Emotional. For each domain there are 11 prompts. For each specific prompt you select where the individual’s functional skills level is currently. The skill level is marked with either an “X” or a checkmark. Once the functional skill level is identified, you then determine if the skill needs improvement and then if it can be improved through Recreational Therapy.

For example, an individual’s hearing may impaired (not correctable). While this may affect program participation, and needs improvement, this is not something that a Recreational Therapist can improve, therefore it will be selected as No under the Can Be Improved thru RT and Needs Improvement category. On the other hand, if someone has a skill level of minor weakness under strength, this is something that can be improved through Recreational Therapy, therefore it will be selected as a Yes under the Can Be Improved thru RT and Needs Improvement category.

Once all sections are complete, you will need to score the assessment. To score the FACTR-R, you tally the number of “No” answers in the Can Be Improved thru RT and Needs Improvement category for each domain. The number of “No” results is the number that will be placed at the top of the assessment for each domain. The goal is to have 11 out of 11 for each domain. A lower number in a domain means the individual needs more attention to that specific domain. A higher number in a domain means the individual is more proficient to that specific domain.

After completion of the initial assessments, the initial assessment instructions and scoring sheet should be uploaded
to the profile of the individual supported on the Bureau of Disabilities Services (BDS) Online Portal.
